Youth Programs
Youth Protection & Background Check
Read the policy, acknowledge it, and authorize a background check. CLT pays for every check (~5–8 minutes).
Health & Safety
Emergency Contact & Medical
Participant contacts, medical notes, OTC permissions, and pick-up authorization for productions and camps.
Need Paper?
Paper blanks (cast/crew agreements, emergency contact, W-9) are available from production staff or by emailing cltplays@gmail.com. Independent contractors: download the official IRS Form W-9 and return it to CLT before payment.
