Policy Acknowledgment & Authorization
The Clemson Little Theatre (CLT) is committed to maintaining a safe, nurturing environment for all youth participants and has zero tolerance for child abuse in any form. This policy applies to all staff, Board members, cast, students, volunteers, contractors, and anyone involved in CLT programs.
Screening & Background Checks. Adults who have any contact with youth while volunteering will undergo a criminal background check and a National Sex Offender Registry screen. Adults with no youth contact are screened through the registry only. CLT pays for all checks and stores results for two years.
Rule of 3. No adult will be one-on-one with a youth in a private space. Doors remain open when brief one-on-one interaction is unavoidable.
Fraternization, Touching & Communication. Transportation, parties, dating, and social-media contact with youth are restricted per CLT policy. Appropriate touching is youth-initiated, observable, and limited to hand, shoulder, or upper back. Adults may not follow or privately message youth on social platforms.
Mandatory Reporting. Report suspected child abuse or neglect to the SCDSS 24-hour hotline at 1-888-CARE4US (1-888-227-3487). Call 911 for life-threatening emergencies, and notify a Producer, Stage Manager, Teacher, Office Manager, Artistic Oversight Member, or Board Member.
Non-Retaliation & Discipline. CLT prohibits retaliation for good-faith reports. Violations result in discipline up to termination and reporting to authorities as required by law.
Authorization. By signing below I authorize CLT and/or its agents to conduct a criminal background check and obtain related information (which may include driving record, arrests, and civil or criminal proceedings). I release from liability all parties supplying such information and CLT’s reasonable use of it, agree to hold CLT harmless, and understand I may request a copy of the report. I understand my SSN is not collected here but may be requested later only if screenings return multiple same-name results.

IRS Form W-9
Because you’ll be paid as an independent contractor, CLT needs a completed IRS Form W-9 on file for tax reporting. This is the official federal form — download it, complete it, and return it with your packet.
