CLT is committed to a safe environment for all youth participants and has zero tolerance for child abuse. Adults with youth contact undergo a criminal background check and National Sex Offender Registry screen (CLT pays; results stored 2 years). Rule of 3: no adult alone with a youth in a private space. Restricted transportation, social media, and personal contact with youth apply. Report suspected abuse to SCDSS at 1-888-CARE4US (1-888-227-3487) or 911 for emergencies.

Authorization. By signing I authorize CLT and/or its agents to conduct a criminal background check and related screening. I release suppliers of that information and CLT’s reasonable use of it. No SSN is collected on this form.
